Volunteers must be over 18 to sign-up. Must be 21 to help with liquor.

By signing below hereby acknowledge and agree to the following:

1. Assumption of Risk: I understand that volunteering at the "Birdies Fore Boobies" charity golf tournament involves certain inherent risks, including but not limited to:

   • Physical exertion and potential for minor injuries.

   • Exposure to outdoor elements, such as heat, sun, and insects.

   • Interaction with golfers and other event participants.

2. Release of Liability: I hereby release and forever discharge Real Roofers Wear Pink & Rock Solid Roofing, its organizers, sponsors, volunteers, and Maumelle Country Club from any and all claims, demands, actions, or causes of action arising out of or in connection with my participation as a volunteer in this event, including any injury, loss, or damage to myself or my property, whether caused by negligence or otherwise.

3. Medical Authorization: I authorize Real Roofers Wear Pink and Rock Solid Roofing to seek medical treatment for me in the event of an emergency during my volunteer service. I understand that I am responsible for any medical expenses incurred.

4. Photo/Video Release: I grant Real Roofers Wear Pink and Rock Solid Roofing permission to use any photographs or videos taken of me during the event for promotional or informational purposes.

5. Agreement to Follow Instructions: I agree to follow all instructions and guidelines provided by event organizers and staff. I understand that failure to do so may result in my dismissal from volunteer duties.

My signature indicates that I have read and fully understand this waiver and release of liability. I am voluntarily participating as a volunteer and assume all risks associated with my involvement.
